Olivia Perks was a 19 year old Officer Cadet at the Royal Military Academy Sandhurst when she was found dead her room on 6 February 2019.

On Friday 26 May 2023 Assistant Coroner Alison McCormack concluded that Olivia died by suicide and that, in the days and months before her death, there had been a number of failings and missed opportunities in the management of Olivia’s mental health by those working at Sandhurst.

The Coroner heard that Olivia’s death and the internal Service Inquiry that followed it had led to wholesale changes to the welfare system at Sandhurst; to the policies and procedures designed to protect vulnerable service personnel, and to the resources available to support them.
